I.         Documents Provided by Employer (Petitioner):

1.        Company brochure or website;
2.        Employer Information Sheet Completed by Employer;

II.        Documents Provided by Employee (Beneficiary):

1.        Resume;
2.        Copy of degree certificate (Bachelor's or higher);
3.        Copy of transcripts (if available);
4.        Copy of passport ID page;
5.        Two sides copy of I-94 (if in the U.S.);
6.        Proof of legal status in the U.S, including visa (s), I-20, I-797, etc. (if in the U.S.);

III.      Documents Provided by Dependents of Beneficiary (if applicable):

1.        Copy of Dependent's passport ID page (if in the U.S.);
2.        Two sides copy of Dependent's I-94 (if in the U.S.);
3.        Proof of legal status in the U.S. (if in the U.S.);
4.        Documents establishing dependent relationship, i.e., marriage certificate for spouse, birth
certificate for child, etc.

IV.      Government Fees

1.        Filing Fee:                                                                        $320.00
2.        Fraud Prevention Fee:                                                     $500
3.        U.S. Worker Training Fee (exempted for schools):         $750 if less than 25 employees

4.        Premium processing fee (Optional):                                $1000
5.        H-4 Dependent Filing Fee (if applicable):                        $300.00
